The short answer
Between 45 and 70 minutes for most of the day. Before 6am or after 10pm you can do it in 35. Between 3pm and 7pm on a weekday it can pass 90 minutes, and on a Friday before a holiday weekend it can pass two hours. Anyone who quotes you a flat 40 minutes has not driven it recently.
Why the range is so wide
Almost all JFK traffic funnels onto the Van Wyck Expressway, a road built in 1950 for a fraction of today's volume. When the Van Wyck is moving, everything works. When it is not, there is no good alternative for the first three miles. Past that, you have choices: the Long Island Expressway to the Queens-Midtown Tunnel is the standard route to Midtown; the Belt Parkway to the Brooklyn-Battery Tunnel serves downtown and often beats the LIE in the afternoon; the Grand Central Parkway to the RFK Bridge works for the Upper East Side and Harlem.
Neighborhood by neighborhood
Downtown and the Financial District are usually the quickest at 40 to 55 minutes via the Belt. Midtown East and Murray Hill run 45 to 65. Midtown West and Hell's Kitchen add ten minutes for the crosstown. The Upper East Side is 50 to 70 via the RFK. The Upper West Side and Morningside Heights are the longest at 60 to 80 because you cross the park or take the West Side Highway.
How much time to leave for a departing flight
For a domestic departure, leave Midtown three hours before scheduled departure. For international, three and a half to four. That sounds excessive until the one time the Van Wyck closes for an incident and you miss a flight to Europe.
